New Preceptor Application
Thank you for your interest in becoming a part of our health care team, and preceptor for NSU's Barry and Judy Silverman College of Pharmacy Experiential Education Program!
We are always seeking qualified preceptors and sites for placement of our students who are eager to learn about the pharmacy profession.
Please complete the online application with the requested information. The application must include the following in order to be processed in a timely manner:
- The Completed Preceptor Application
- Current Pharmacy License
- CV (to be uploaded)
- Syllabi (to be uploaded)
* If you request to be a preceptor at a REQUIRED pharmacy practice experience, you only need to check the radio button on the application affirming that you will follow NSU's Barry and Judy Silverman College of Pharmacy syllabus, including goals, objectives, and grading format.
Upon receipt of your complete packet (including all uploaded documents as listed above), you will be contacted by a faculty member to set up a site visit to meet with you as well as any other preceptors at your site. Please note -- if your store, pharmacy, hospital etc., has a current Affiliation Agreement with NSU's Barry and Judy Silverman College of Pharmacy you will only need to be added as a preceptor. If there is no established affiliation agreement with the Silverman College of Pharmacy, the process will take longer to complete. The application process may take anywhere between 3 to 6 months.
