In addition to the Puerto Rico Entry Level Application, below are the additional items necessary to complete your application before it can be reviewed by the Committee on Admissions.

- An official transcript from the registrar of all colleges and universities attended . This must be mailed directly Nova Southeastern University Pharmacy Admissions Office.
- PCAT Scores (no more than five year old)
- A non-refundable application fee.
- 3 Letters of Recommendation- Two letters from science professors and one from a liberal arts professor. A letter of recommendation from a pharmacist who is a nonfamily member may substitute for a letter from a professor.

Interviews |
A personal interview is part of the admissions process; however, being interviewed is not a guarantee of admission. Not all applicants will be granted and interview. Those selected for an interview will be notified of the date and time of such interview by the Office of Admissions.
Notice of acceptance or action by the Committee on Admissions will be on a "rolling" or periodic schedule; therefore; early completion of the application is in the best interest of the student. |
Transcripts

After acceptance, final transcripts from all colleges and universities attended must be forwarded to the Office of Admissions prior to matriculation. It is the responsibility of the applicant to ensure that arrangements are made for these transcripts to be sent. |
